摘要目的 探讨帕金森病伴认知功能障碍(PD-CI)患者各网络间的功能连通性及意义。方法 采用独立成分分析(ICA)方法对 34 例 PD 患者与 34 名健康对照者(HC 组)、20 例 PD-CI 患者与 HC 组两两进行全脑独立成分分解,以独立成分为单位,计算独立成分之间的功能网络连通性(FNC)。结果 与HC 组相比,PD 患者的感觉运动网络与右侧额顶网络、左侧额顶网络与默认模式网络的功能连接减弱,感觉运动网络(11)与感觉运动网络(5)、腹侧注意网络与感觉运动网络、腹侧注意网络与视觉网络(6、25)、视觉网络(6)与视觉网络(25)、视觉网络(6、25)与听觉网络、感觉运动网络与听觉网络、腹侧注意网络与背侧注意网络(32)的功能连接增强。与 HC 组相比,PD-CI 患者的左侧额顶网络与默认模式网络的功能连接减弱。结论 PD 患者脑区间功能连接存在广泛异常,而 PD-CI 患者的左侧额顶网络与默认模式网络的功能连接减弱,认知相关脑网络之间连接存在一定程度的受损。

AbstractObjective To explore the functional connectivity and its significance between networks in patients with Parkinson’s disease with cognitive impairment(PD-CI). Methods The independent component analysis(ICA)method was used to decompose the whole brain independent components of 34 PD patients and 34 healthy controls(HC),and 20 PD-CI patients and 34 healthy controls in pairs. The functional network connectivity(FNC)between the independent components was calculated. Results Compared with HC group,the functional connectivity between sensorimotor network and right frontoparietal network and left frontoparietal network and default mode network in PD patients was weakened. The functional connectivity between sensorimotor network (11)and sensorimotor network(5),ventral attention network and sensorimotor network,ventral attention network and visual network(6,25),visual network(6,25)and visual network(25),visual network(6,25)and auditory network,sensorimotor network and auditory network,ventral attention network and dorsal attention network(32) were weakened. Compared with HC group,the functional connectivity between the left frontoparietal network and the default mode network in PD-CI patients was enhanced. Conclusion There are extensive abnormalities in the functional connectivity of the brain intervals in PD patients,while the functional connectivity between the left frontoparietal network and the default mode network in PD-CI patients is weakened,that the connectivity between the cognitive related brain networks is damaged to a certain extent.
