摘要目的 研究创伤性中型颅脑损伤后认知障碍现状及其影响因素。方法 纳入 2020-09—2021-09 于联勤保障部队第九九〇医院神经外科住院治疗的创伤性中型颅脑损伤患者 100 例为研究对象,患者入院 1 周后应用简易精神状态量表评定患者认知功能情况,存在认知功能障碍患者为障碍组(n=73),无认知功能障碍患者为无障碍组(n=27),对 2 组患者基础资料进行单因素分析,对认知障碍影响因素进行多元线性回归分析。结果 障碍组与无障碍组性别、饮酒史与吸烟史比较差异无统计学意义(P>0.05)。2 组年龄、受教育程度、损伤位置、多部位损伤及精神状态评分等比较差异有统计学意义(P<0.05)。年龄、额叶损伤、多部位损伤是影响创伤性中型颅脑损伤患者出现认知障碍的主要因素。结论 创伤性中型颅脑损伤早期即存在广泛的认知障碍,创伤性中型颅脑损伤当中多部位损伤、额叶损伤以及患者年龄等均是影响认知障碍的重要因素。

Status and influencing factors of cognitive impairment after traumatic moderate craniocerebral injury

AbstractObjective To investigate the status and influencing factors of cognitive impairment after traumatic moderate craniocerebral injury. Methods A total of 100 patients with moderate traumatic brain injury hospitalized in the Neurosurgery Department of 990th Hospital from September 2020 to September 2021 were included. One week after admission,the cognitive function of the patients was assessed by the simple mental state scale. After the evaluation,a total of 73 patients with cognitive dysfunction were classified as the impairment group. Twenty-seven patients without cognitive impairment after craniocerebral injury were divided into the barrier-free group. Single factor analysis was used to compare the basic data of patients in the two groups,and multiple linear regression analysis was used to analyze the influencing factors of cognitive impairment in the two groups. Results There were no significant differences in gender,drinking history and smoking history between the two groups(P>0.05). Age,education level,location of injury,multiple site injury and cognitive impairment scores were significantly different(P<0.05). Age,frontal lobe injury and multi-site injury were the main factors affecting cognitive impairment in patients with traumatic brain injury. Conclusion There is extensive cognitive impairment in the early stage of traumatic moderate brain injury. Multiple site injury,frontal lobe injury and age of patients are important factors affecting cognitive impairment.
